Department of Anaesthesiology and Resuscitation

The ARO has fully equipped beds where it provides care to patients after all surgical procedures performed at the Hořovice Hospital. It provides all types of general and regional anaesthesia in central operating theatres. If necessary, it provides intensive post-operative monitoring or specialised post-operative care, e.g. using elimination methods. It ensures the operation of an anaesthesiology outpatient clinic and comprehensive pre-operative preparation of patients for planned and acute procedures, as well as treatment of acute post-operative pain.

Profile of the anaesthesiology section of the ARO department

The anaesthesiology department provides comprehensive care for patients throughout the period before, during and after a surgical or diagnostic procedure requiring anaesthesia or analgosedation. A wide spectrum of general, local and general anaesthesia is performed at Hořovice Hospital. The ARO department deals with the treatment of acute and chronic pain. Currently, the department provides anaesthesia services to all surgical disciplines. To increase the comfort and safety of anaesthesia, pre-anaesthetic examination for elective surgical and diagnostic procedures is performed in the anaesthesia outpatient department.

Job description:

  • Operation of the anaesthesiology outpatient clinic - comprehensive pre-operative preparation of patients for elective and acute procedures.
  • Provision of all types of general and regional anaesthesia in central operating theatres.
  • Provision of obstetric analgesia and anaesthesia for caesarean sections and other acute obstetric procedures.
  • Provision of outpatient anaesthesia and anaesthesia for short procedures at detached departments.
  • Ensuring the operation of the recovery room in the COS campus and necessary consular activities within the NH.
  • Provision of acute post-operative pain management.

Resuscitation section

The Resuscitation Department deals with complex diagnostics and treatment of critically ill patients, i.e. such patients whose life is threatened due to impairment of basic life functions. In our hospital, the care of critically ill patients is integrated into a multidisciplinary intensive care unit (ICU), which includes, in addition to the ARO beds, intensive care units of the surgical specialties, internal medicine and neurology.

The ARO has 5 fully equipped beds where it provides care for patients:

  • after all surgical procedures performed in the NH, in case of need for intensive postoperative monitoring, in case of complications of anaesthesia or surgical care, in case of need for specialised postoperative care, e.g. use of elimination methods
  • patients hospitalised or treated in the NH (outpatient department, standard ward, ICU) with acute deterioration of general condition requiring resuscitation care, patients after successful cardiopulmonary resuscitation
  • in the catchment area, brought to the ambulance, requiring resuscitation care or specialised ARO care (elimination methodologies)
  • catchment, transferred by arrangement from other health care facilities, requiring resuscitation care
  • non-catchment (in exceptional cases) and after prior agreement with the head of the ward or the head doctor of the inpatient ARO.

Structure of care and types of procedures performed in the ARO:

  • cardiopulmonary resuscitation for the whole NH
  • artificial pulmonary ventilation, including non-invasive pulmonary ventilation, with the possibility of monitoring lung mechanics parameters
  • Comprehensive circulatory support and monitoring, including the use of all available pharmacological techniques, invasive monitoring with a pulmonary artery catheter and the transpulmonary thermodilution method (PiCCO). Temporary external and transvenous cardiac pacing (also performed for patients in the internal medicine department), now also with the possibility of insertion under the skiascopic control in the X-ray department.
  • Care of patients with cerebral oedema or craniocerebral injury not requiring acute neurosurgery or intracranial sensor insertion
  • Comprehensive care of the internal environment and nutrition, with the exception of insertion of enteral probes into the GIT
  • replacement of renal function. Continuous elimination methodologies - CVVH, CVVHD, CVVHDF, the device also allows performing hemabsorption
  • treatment of intoxications
  • complex pharmacotherapy, targeted ATB therapy in cooperation with a microbiologist

Performing invasive procedures:

  • tracheal intubation
  • percutaneous dilatational tracheostomy
  • insertion of central venous catheters, arterial catheters, dialysis catheters, pulmonary artery catheters, PiCCO catheters, transvenous pacing
  • performing chest drains
  • insertion of permanent urinary catheters and nasogastric probes

The anaesthesiology and resuscitation department is subject to a special hygiene and operational regime.

The recommended visiting hours are daily from 14.00 - 16.00. Visits outside these hours must first be arranged by telephone or in person with the attending physician or nurse.

A maximum of 2 adults may visit the ward at the same time, children are not allowed. In the case of a larger number of people, it is necessary to take turns.

Before entering the ward, you will put on a protective gown (empire) and wear disposable shoe covers. If you are currently experiencing a communicable respiratory infection or have another communicable disease, please consult with our ward doctor to enter the ward. The risk to you and the patient must be considered.

On arrival, wait in the admission filter for the nursing staff to usher you to the patient box. If the patient is currently undergoing an investigation, instrumental procedure or other important activity, you will be informed of the time at which you will be able to see the patient.

If you wish to bring anything to the patient, especially food, you must first make arrangements with the nursing staff.

Providing health information

In accordance with applicable laws, we only provide information about a patient's health status to loved ones or court-appointed guardians. A close relative is a relative in the direct line of descent (parents, grandparents, children, grandchildren), sibling, spouse and partner (according to the Registered Partnership Act).

We only provide information to verifiably identified persons, initially only on face-to-face contact during the visit, and subsequently a 'password' can be agreed for telephone reporting. We are unable to provide health information to previously unknown persons.

Specifikace poskytované péče:

Ambulance pro léčbu chronické bolesti se zabývá diagnostikou a léčbou chronických bolestivých stavů. Věnuje se léčbě bolesti z různých příčin – bolesti nenádorové i nádorové.

  • Pacienti: Hlavní skupinu pacientů představují pacienti s chronickou bolestí zad, bolestmi kloubů, bolestí hlavy a bolestí neuropatickou doprovázející různá další onemocnění (cukrovka, pásový opar, ischemická choroba dolních končetin, stavy po chemoterapii a podobně).   • Metody: V Ambulanci léčby bolesti jsou využívány k léčbě bolesti metody farmakologické i nefarmakologické. ALB disponuje širokým spektrem léků proti bolesti (analgetik) a pomocných léků (koanalgetik) s možností různého způsobů podání léku. Další součástí terapie v ambulanci je možnost podávání analgetických infuzí a provádění invazivních metod – nervových blokád (opichů) včetně kontinuálních metod (v rámci polodenního stacionáře). Lékař ambulance bolesti poskytuje konziliární služby pro celou nemocnici v rámci odbornosti léčba bolesti a paliativní medicína.

  • Spolupráce: Ambulance pracuje v návaznosti na lůžkové resuscitační oddělení, konziliární ambulantní a lůžkové neurologické oddělení, rehabilitační oddělení, ambulantně multidisciplinární spolupráce s odborností interní, kardiologickou, psychologem, psychiatrem, revmatologem. Kompletní laboratorní zázemí a zázemí v oblasti zobrazovacích metod (RTG,CT, sono, ECHO) i s možností provádění intervenčních metod pod CT,RTG, sonografickou kontrolou. Ambulance má přímou návaznost na vyšší pracoviště ALB IKEM Praha.
