On VZP health insurance form 06-ft - Voucher for examination/treatment of FT at physiotherapists' workplace
On this form, the attending physician writes a request for specific treatments performed by a separate physiotherapists' office. The patient can choose which department to visit. In the Hořovice Hospital you will find such a workplace in the basement - Department of Physiotherapy. You can make an appointment by phone, better with a form in person (the physiotherapist can check what procedures are required by the prescribing doctor and adjust the schedule of required procedures accordingly).

On this form, the attending physician writes a request for examination and treatment by a rehabilitation medicine specialist or a request for hospitalisation in a specialised inpatient medical rehabilitation unit.
You can use this form to make an appointment for examination/treatment at the specialist's office of Hořovice Hospital or Rehabilitation Hospital Beroun by phone.
If your attending physician requests hospitalization in a rehabilitation unit on the form, please contact the office of the Rehabilitation Center Beroun by phone.

Make an appointment to be seen by a physician specializing in medical rehabilitation. FT (see attached pdf file). You can choose a specialist of the Hospital Hořovice or a specialist of the Rehabilitation Hospital Beroun.
