Palliative and support team
Our palliative care is provided by a multidisciplinary team of nurses, physicians, social workers, psychologists and chaplains.
We strive to be a good guide to the sick and their loved ones in a difficult life situation. Our goal is to provide care according to the wishes and capabilities of the patient and family. Our job is to communicate with the patient his/her medical condition, diagnosis and prognosis if he/she wishes, and an integral part of our work is to manage the pain and other physical difficulties that a terminal illness brings.
Together with the patient and family, we draw up a care plan, help navigate the health and social system options and organise quality care at home or in an inpatient facility.
Outpatient care
In the palliative care outpatient clinic, we identify the needs of the patient and family, plan further care, meet patients for check-ups, but also arrange home visits by a nurse or physician according to the patient's needs and organise care at home in collaboration with home care agencies
Concierge services
We work closely with colleagues in all departments of Hospital Hořovice, together we care for patients in the form of a consular service to provide the highest possible quality of life and create a care plan acceptable to the patient and their family. Patients or their relatives can request the involvement of the palliative care team in their care by contacting the attending physician in each ward.
Symptomatic care
We help to set up treatment for the unpleasant symptoms that may accompany the illness - shortness of breath, indigestion, anxiety , depression restlessness and other problems.
Psychological care
We can provide both rapid crisis intervention and ongoing support for patients and their loved ones, including care for patients' children or for seriously ill children themselves
Social counselling
We concentrate on providing follow-up care in the home environment (home health care, home hospice care, support services or outreach social services) or institutional care (hospice, aftercare beds, respite/relief stays). We help you to navigate the system of social benefits and financial support, as well as planning and providing compensatory aids.
Spiritual support
We offer a space for sharing, psychological support and guidance for patients, loved ones and care staff with respect for personal beliefs. Hospital chaplains can support with conversation, silent presence, prayer.
Counselling
Bereavement counselling develops the possibility of continuity of care for the bereaved, we would like to support them through the difficult period following the death of a loved one
