A laparoscopic trainer is a special device that simulates the conditions of a real laparoscopic operation and allows surgeons to practice various techniques that are a common part of many procedures. It is used by young surgeons to train precision manipulation, movement coordination and fine motor skills, as well as surgical skills such as laparoscopic suturing. "They thus achieve technical proficiency before they proceed to the actual surgery. And their practical training is more effective, because they master the use of laparoscopic devices faster than when we teach them in the operating room," says Martin Oberreiter, MD, Deputy Director for Medical and Preventive Care and Head of the Surgical Department at Hořovice Hospital.
The laparoscopic trainer is equipped with its own camera and monitor. Thanks to the movable camera, it is possible to change the angle of view of the surgical field during training as needed. In the hospital in Hořovice, the simulator is located in the surgeons' day room and can be used for training by both young doctors and graduates. "It is a great bonus. While still at the faculty, I did my internship in Prague hospitals and the laparoscopic simulator is definitely not among the standard equipment even at higher professional workplaces," says surgeon Václav Nesrsta, MD.
He positively evaluates that he can use the simulator regularly and continuously improve his fine motor skills, dexterity and coordination of movements necessary for laparoscopic procedures. "It is a great advantage that we can repeatedly practice the control of laparoscopic instruments before we go to the operating room for the first time," adds young doctor Lucie Kopecká, MD.
Nowadays, almost all organs in the abdominal cavity are operated on laparoscopically. Among the most common laparoscopic procedures are gallbladder, abdominal hernia or appendix operations. There are a number of advantages for patients with this method, including smaller incisions and faster recovery.
Hořovice Hospital plans to purchase another laparoscopic trainer this year. "By the end of the year, we will also purchase a robotic system. It is another of the hi-tech conveniences that will take the art of surgery a little further. We already have certified doctors capable of operating with the surgical robot," adds MUDr. Martin Oberreiter.
Laparoscopy is now a standard, very gentle surgical method in which the doctor introduces special surgical instruments into the patient's body through small incisions. The key element of laparoscopy is the surgeon's precision and skill with the instruments, as well as a camera that transmits a detailed image from inside the body to a monitor, allowing surgeons to work with high precision.
The robotic system is controlled by the surgeon during surgery using sensitive joysticks. They control the robotic arms with instruments. The device simultaneously displays a three-dimensional magnified image of the operated area. The instrument arms are supported by a powerful computer and are therefore highly mobile and their movement, controlled by the surgeon, is extremely precise. The procedure is minimally invasive for the patient and the recovery time is even shorter than with laparoscopic procedures.
