Hořovice ENT expands the range of outpatient and surgical procedures

Hundreds of patients a year seek the services of the ENT clinic, which is very active in Hořovice. In the following interview, MUDr. Daniela Olšavová introduces the outpatient and surgical procedures offered by ENT and presents plans to expand the care provided.

Doctor, can you describe the concept of ENT in our hospital?

ENT works in our hospital in two ways, outpatient and surgical. We have 2 ENT outpatient clinics providing comprehensive diagnosis and care for adult and pediatric patients with head and neck diseases. One outpatient clinic is a general outpatient clinic, focusing on "common" ENT problems. The other outpatient clinic is specialized, with modern instrumentation including rigid and flexible endoscopes. We also have a newly installed video chain. It is primarily focused on consultations and examinations of patients indicated for surgical procedures, complex diagnostics of patients with sleep-disordered breathing, consultations for individual departments, etc. Outpatient surgical procedures are performed here. The ENT department also includes the sleep outpatient clinic, where conservative treatment of patients with already diagnosed sleep apnoea syndrome is carried out.

What surgical procedures do you perform on an outpatient basis?

We perform radiofrequency (RF) ablation procedures on an outpatient basis, namely uvuloplasty - surgical treatment of snoring, turbinoplasty - reduction of the volume of the nasal mucosa. In addition, we perform procedures in the oral cavity, most often removal of mucosal neoplasms, cutting of shortened sublingual glands, removal of mucoceles, removal of sialoliths from the salivary gland outlet and others. At the ENT outpatient clinic we have a surgery room where we perform procedures under local anaesthesia, especially removal of skin lesions on the face and neck, mainly malignant or benign skin tumours, pigmented nevi, warts and other skin formations. We also perform the application of earrings classically with a needle or by injection.

What kind of procedures is ENT surgery under general anaesthesia focused on?

As we do not have a separate ENT department, our patients are admitted to the surgical or paediatric ward. In children, we operate on adenoid vegetations (endoscopic adenotomy) and perform correction of protruding pinnae (otoplasty). For adult patients, we focus almost exclusively on rhinosurgery (nose surgery) and surgical management of snoring. We perform septoplasty (surgery of deviated nasal septum), FESS - endoscopic sinus surgery, where we operate on nasal polyps, tumors and chronic inflammation, or rhinoplasty (surgery of deformities of the external nose). As part of the surgical treatment of snoring, we perform uvuloplasties and uvulopalatoplasties by RF ablation.

How long are patients hospitalized after surgery?

As a standard, we discharge paediatric patients on the day of surgery, while adult patients are hospitalized for a total of 3 days.

What new developments are you planning for the future of ENT?

We have future plans to perform sonographic examination of the neck and fine needle aspiration biopsy, called FNAB, for diseases of the salivary glands, thyroid gland and lymph nodes in the neck. We plan to expand surgery under local anesthesia, especially in the area of facial aesthetic surgery, and perhaps expand surgery under general anesthesia.
