Health information for the patient's relatives

Health information is confidential and we may only disclose it with the patient's consent to persons designated in writing by the patient at the time of admission to the hospital in the Consent for Hospitalization. It will be provided by the attending physician in the afternoon or by prior arrangement with the ward nurses.

Information form for patients with dementia or similar illness

This form is for family members of patients with dementia or a similar illness.
It is advisable to have a completed form ready in case the patient is transferred to a healthcare facility.
The information will help us during admission to hospital.

Where to come for a reception?

On the day of your scheduled appointment, come to the internal medicine department in the morning (unless otherwise arranged).

I'm going in for surgery

For more complex diagnostic or therapeutic procedures, you will be asked to give written consent on the Informed Consent for Inpatient Admission.docx form. This will give you general information about the operation or procedure, its benefits and possible complications. The attending physician will provide you with a detailed explanation and the individual course of treatment. Procedures for which written informed consent is required:

Room facilities

Each room is equipped with toilet, sink and shower, TV, wardrobes.
3 rooms with 4 beds, 4 rooms with 3 beds and 4 extra 1-bed rooms.

Health information is confidential and we can only disclose it with the patient's consent to persons designated in writing by the patient on admission to the hospital in the Consent for Admission form. It will be provided by the attending physician in the afternoon or by prior arrangement with the ward nurses.

What to bring?

- ID card or other proof of identity (passport)
- health insurance card
- referral for hospitalisation and results of previous examinations
- disability card (if you have one)
- personal belongings:
pyjamas (nightwear)
dressing gown
slippers (non-slip, comfortable, without heels)
personal hygiene items
towel
if you need anything special, you can find out from the ward staff and on the ward website.
- Medicines: during your stay in hospital, all medicines are prescribed by the attending doctor and administered by the nurse. If you are taking certain medicines to treat a specific condition, take them with you to the hospital and give them to the nurse when you arrive on the ward. The attending physician will include them among the medications that will be given to you during your hospital stay. Do not take any medication during your hospital stay without the doctor's knowledge. You will receive your medication from the nurses in the exact dose and at the exact time.

Do not carry: large sums of money, valuables, weapons.
There is a wardrobe for storing clothes during your stay in the hospital; items of daily personal use can be stored in the table next to the bed. Valuables, jewellery, larger sums of money and important documents are recommended to be stored with the nursing staff.

Visit

Visits to the department

Visits are possible every day due to the treatment regime and the ward regime, we recommend visiting between 14.00 and 18.00 hours. Please note that visitors may be asked to leave the patient's room in the case of bedside procedures, personal hygiene or medical appointments. Visitors under 10 years of age are not appropriate.

In the case of isolation of the patient due to an infectious disease, the visitor is required to equip himself/herself with personal protective equipment (drape, gloves, empire) at the room according to the instructions of the ward staff.

In case of hospitalization of the patient in the intermediate care room (IMP), visits of maximum 2 persons older than 10 years are possible. Please respect the staff's recommendation on the appropriate length of visit.
